We ask that you put this request in writing to Info@mosmansmiles.com.au and our practice will send a consent form for your dental records and once the form has been received from you, our practice will respond within a reasonable time (<30 days).

From time to time, we will ask you to verify that your personal information held by our practice is correct and current.
